You'll provide in-home health services across both infusion and home health cases. Key Responsibilities * Administer services requiring specialized infusion skills * Manage medication routines, including reconciliation and education * Provide trach care, wound care, and enteral feeding tube support * Monitor health status and respond to clinical changes * Maintain accurate and compliant documentation * Deliver care based on individualized Plans of Care * Educate patients and families to promote confidence and independence This position includes travel within a 30-mile radius and offers meaningful, hands-on experience across a wide range of needs. What We're Looking For * A current, unrestricted California RN license * Experience with IVs, ports, PICC lines, and home health procedures * Strong clinical skills in trach care, enteral feeding, wound care, and med management * Proficiency in documentation and care planning * Familiarity with HIPAA regulations and Joint Commission standards * Reliable transportation and valid auto insurance * Clear background check Advanced certifications like CRNI or OCN are a plus.
