The Social Worker is responsible for providing medical social casework services for home care patients in accordance with compliance with organization policies and procedures and applicable laws and regulations. Schedule: Monday - Friday; 8:30am - 5:00pm Location: San Francisco & Marin Counties By the Bay Health follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to): * Evaluates the need for medical social casework services to patients in their place of residence * Participates in ongoing interdisciplinary assessment of the patient * Assesses social and emotional factors relating to the patient's condition to estimate the capacity of the patient and family to cope with problems of daily living * Participates in ongoing interdisciplinary assessment of the patient * Accurately assesses the need for, and performing, revisits * Accurately assesses the need for and initiating referrals to community resources Education/Experience: * Master’s degree in social work from a school accredited by the Council on Social Work Education or Master’s degree in mental health discipline * Minimum one-year’s social work experience in a health care setting Pay Range: $44.50 - $52.30 hourly By the Bay Health is proud to serve our community as an equal employment opportunity employer.  Everyone is valued and welcome at BTBH.
