**$15,000.00 Retention Bonus Available** We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in the Springfield, MO and surrounding area. This is a Full-Time position.  Reduce stress and burnout!  Provide clinical care where patients want to be – in the comfort of their own home. **$15k Retention Bonus is for Full Time only** Why you’ll love what you do at Integrity Home Care + Hospice: * Experience true work/life balance with flexible full-time, part-time, and per diem schedules * Perform 1:1 direct care in client homes * Hands-on training supplemented with virtual learning * 24/7 clinical support as part of a larger interdisciplinary care team * Advance your career with clinical and business leadership tracks What you’ll do: Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.   Responsibilities for a PT - Physical Therapist include: * Make home visits to clients in designated geographic territories. * Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. * Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.  * Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. * Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate. * Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living. * Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.   What you should have: * Active and current PT license * One year of clinical experience preferred * Previous home health care experience welcomed, but not required.
