Associate Care Manager (MA/CNA Level) – Babyscripts About Babyscripts Babyscripts is a leading innovator in digital health, dedicated to transforming maternal care through remote patient monitoring (RPM) and patient engagement. Our digital tools help healthcare providers monitor, educate, and support their patients in real time, enhancing overall care coordination and improving health outcomes. About the Role We are seeking a full-time Associate Care Manager to support patient engagement, care coordination, and remote patient monitoring efforts. This role is ideal for candidates with a background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role with experience in care coordination, patient navigation, or remote patient monitoring (RPM). The Associate Care Manager works closely with patients enrolled in Babyscripts' RPM programs, ensuring compliance with care plans, addressing questions, and escalating concerns to clinical staff when needed. While prior experience in maternal health or OB care is a plus, it is not required. The ideal candidate is passionate about patient engagement, digital health, and improving healthcare outcomes. This is a remote position with structured yet flexible scheduling, offering an opportunity to make a meaningful impact on maternal healthcare while working in an innovative and fast-growing digital health company. Key Responsibilities * Engage with patients through in-app messaging, phone calls, and telehealth check-ins to support adherence to their care plans. * Monitor patient data (such as blood pressure readings) and conduct follow-ups with patients who have not submitted readings in several days. * Educate patients on how to use digital health tools and properly measure vital signs. * Escalate concerns related to abnormal patient readings or reported symptoms to RN Care Managers or clinical teams based on established protocols. * Provide care coordination support, including appointment scheduling and answering routine patient questions. * Assist in patient onboarding for Babyscripts' RPM program, ensuring patients understand expectations and have access to necessary resources. * Document all interactions accurately in Babyscripts' care management platform. Preferred Qualifications & Experience * Experience in care coordination, patient engagement, or remote patient monitoring (RPM) is highly preferred. * Background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role. * Strong communication and interpersonal skills, with a patient-first mindset. * Proficient with digital health tools, EHR systems, and telehealth platforms. * Ability to work independently in a remote environment while maintaining productivity. * Comfortable with structured but flexible scheduling, ensuring full-time work commitment. * Bilingual (Spanish preferred) is a plus. Compensation & Benefits * Flexible PTO Policy: Babyscripts offers flexible paid time off for full-time employees, ensuring a healthy work-life balance while maintaining coverage and performance expectations. Employees are encouraged to take at least 3–4 weeks off per year and must meet job performance expectations to qualify for discretionary time off. * Performance Expectations for Flexible PTO Eligibility: * Must maintain full-time status (average of 40 hours/week over a rolling 3-month period). * PTO requests must be submitted in advance to ensure team coverage. * Employees must meet engagement and documentation benchmarks to continue qualifying for discretionary PTO. * Bonus Potential: Employees are eligible for quarterly bonuses based on patient engagement, quality benchmarks, and documentation accuracy, with an annual bonus potential of up to $2,500. * Flexible Scheduling: Employees must maintain expected work hours while having flexibility in when they work based on business needs. * Full-time employees receive: * Medical, dental, vision coverage * 401(k) options * Work-from-home stipend Staffing & Coverage Expectations * Employees are expected to maintain a full-time work schedule (40 hours per week). * PTO must be planned in advance to ensure adequate team coverage. * Coverage for patient interactions is a priority—employees should communicate availability adjustments ahead of time to allow for rescheduling. * In cases of urgent or last-minute PTO, employees should work with their manager to coordinate appropriate patient handoff procedures.
