Behavioral Health Care Manager role: The BHCM is a core member of Cerula Care’s collaborative care team. Cerula Care’s team includes a BHCM, a Consultative Psychiatrist, and a Patient Engagement team. The BHCM is critical to collaborating between external specialists (e.g., oncologists) and the internal care team; the role is partly clinical and partly operational. Key Traits: * Experienced and passionate about supporting individuals impacted by cancer and chronic illness, with a demonstrated ability to build rapport, engage patients through telehealth, and foster meaningful therapeutic relationships. * Exceptionally organized, accountable, and proactive, with the ability to independently manage a caseload, maintain timely documentation, and effectively coordinate patient care in a fast-paced environment. * Highly adaptable and energized by a startup environment, combining empathy with accountability and taking pride in delivering exceptional patient care while achieving strong performance outcomes. Key Responsibilities: * Engage in an initial clinical member biopsychosocial intake including a safety screen and administering behavioral health assessments (PHQ-9, GAD-7, etc.) * Engage in follow-up visits and asynchronous interactions, including brief interventions (e.g., behavioral activation, mindfulness, psychoeducation, etc.) * Be the key care coordinator and act as a liaison between the referring specialist and the practice, ensuring if there is a member question or administrative question, it is answered or triaged to the appropriate team within Cerula Care * Lead the interdisciplinary team meetings on a weekly basis with the Consulting Psychiatrist and Health Coach, ensuring all high-risk members are discussed and all new psychiatric recommendations are documented in the chart * Communicate closely on an ongoing basis with the Cerula Care behavioral health care team (Coaches and Psychiatrist) * Document member progress in a HIPAA-compliant electronic medical record system and client registry provided by Cerula Care * Identify patients who are not improving and may need more intensive mental health care and report any concerns to the medical provider and the Consulting Psychiatrist * Facilitate referrals for clinically indicated services outside of Cerula Care under the supervision of the BHCM Lead (e.g., SMI). * Support patients in identifying barriers to care, increasing engagement, building self-management skills, and implementing behavioral health treatment recommendations. Educational, Certification, and Experience Requirements: * Required: * Master's degree in Counseling, Clinical Mental Health Counseling, Social Work, Marriage and Family Therapy, Psychology, or a closely related behavioral health field. * MUST be fluent in both English and Spanish, with the ability to conduct clinical conversations, assessments, and documentation-related communication in both languages. * At least three years of clinical care management experience, with some part of the experience directly working with people with cancer or advanced illness (e.g., cancer coaching, palliative care, etc.) * Experience conducting behavioral health intakes, psychosocial assessments, and treatment planning. * Experience working with individuals managing chronic medical conditions, serious illness, cancer, or other complex health concerns. * Experience utilizing evidence-based interventions, including Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), Behavioral Activation, psychoeducation, and coping skills development. * Preferred: * Active or associate-level licensure (e.g., LPC-A, LGPC, LMSW, LSW, LMFTA, MHC-LP, or equivalent). * Experience in oncology, integrated care, collaborative care, behavioral health care management, primary care, or telehealth settings. * Experience delivering brief, goal-oriented interventions within a measurement-based care model.
