Position Summary The Transitional Aged Youth Health & Wellness Case Manager outreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains such as housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and much more.
