Home Visiting & Assistant Community Health Nurse - Internal Posting Full-Time Permanent  Job Summary:  Under the direction of the Home and Community Care Coordinator, the Home Visiting and Assistant Community Health Nurse leads client assessments, care planning, nursing care, and outcome evaluations. They support Biigtigong Mno-zhi-yaawgamig’s continuum of care for vulnerable community members and contribute to community health promotion and injury/illness prevention programs. This role involves dividing time between these programs to improve overall health outcomes through client support, prevention programs, and collaboration with health teams and agencies. Preferable skills & Qualifications: Education and Experience: * Bachelor of Nursing degree from Canadian University or recognized equivalent; or * Diploma in Nursing from recognized educational institution with a minimum of three years’ experience in home care settings * Current foot care certification- basic and advanced or willing to obtain * Experience working in indigenous community Necessary Knowledge: * Ability to work in a manner respectful of Biigtigong Nishnaabeg’s culture, values, and beliefs * Knowledge of indigenous home care issues, health services, and health systems * Knowledge of working within a client/family centered approach * Ability to provide clinical supervision of skills to unregulated health care providers * Ability to ensure confidentiality of client data * Computer skills and Microsoft Office applications Developed Competencies: * Teamwork and collaboration with others; communicate openly, foster strong working relationships and contribute to group efforts * Self-motivated; able to work both independently and as part of a team * Adaptable; able to adjust to changing circumstances, new tasks and unexpected challenges * Time management; able to prioritize tasks, manage workload, and meet deadlines * Professionalism; able to demonstrate integrity, ethics, and respect for others in all interactions and behaviors Duties & Responsibilities: The Home Visiting and Assistant Community Health Nurse shall: * Demonstrate respect for indigenous culture, values, beliefs, and actively integrate traditional knowledge into care plans * Provide comprehensive care addressing physical, mental, spiritual, and social needs, developing client-centered care plans * Provides foot care for common foot and nail disorders or high-risk clients * Conduct thorough client assessments, deliver prescribed treatments, and perform specialized nursing procedures within scope of practice * Organize and implement community workshops, events, and programs focused on health promotion and illness prevention, with the Community Health Nurse * Provide client and family education, and offer guidance to volunteers and students * Implement and maintain safety protocols, including infection control measures, and address client safety concerns proactively * Maintain accurate records, submit timely reports, and manage program data/statistics * Work effectively with the healthcare team, community members, and external agencies, and advocate for clients’ needs * Implement best practice standards, address ethical issues, and comply with relevant regulations and accreditation requirements * Engage in reflective practice, attend training, and contribute to ongoing quality improvement efforts * Participate in meetings with healthcare team and external professionals * Ensure work life and physical environment that supports the safe delivery of care and service to clients Closing Date:  September 14, 2026, at 12:00pm (Noon)
