As a Referral PCC, you will be the first point of contact for patients referred to the company by our trusted network of partners, and you'll also manage the outbound side of referrals - coordinating when the company patients need to be referred out to partner organizations for services outside our scope of care. This is a patient-facing role that blends administrative efficiency, sales acumen, and cross-organizational coordination to keep referrals moving smoothly in both directions.
