Part time Registered Nurse Case Manager Anticipated Territory : Sacramento County Schedule: Every Saturday and Sunday Full Days. Would consider Saturday/Sunday plus 3 week days.     What you’ll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: * Conduct Start of Care (SOC) Admissions - OASIS * Conduct follow ups, evaluation visits as needed in designated territory * Make home visits to patients in designated territory * Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures * Monitor clients’ conditions, reporting changes to Clinical team * Collaborate with physicians and agency staff to deliver the best care possible * Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols. * Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Generous PTO, Medical, Dental, Life Insurance, etc * Competitive compensation with achievable incentive bonus program TO BE A SUCCESS: * Current and unrestricted CA RN license * Current BLS/CPR Certification * Valid CA Driver's License * Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired * High level of clinical knowledge in home health care is highly preferred * Experience with high acuity patients is a plus * Experience working with an interdisciplinary team is highly desired * Case Management fundamentals is highly desired * Previous experience with IV Medication infusions highly preferred * Proficiencies initiating intravenous (IV) therapy is a plus * Self-Directed * Empathetic * Critical thinker * Outstanding interpersonal skills * Leadership skills * Organized 21st Century is an equal opportunity employer, committed to fostering a diverse and inclusive workplace.
