TALENT: * Eager, determined to achieve success, and committed to making a difference every day * Compassionate and care deeply about our patients and their experience * Love what you do and where you work * Not afraid to ask questions and grow as a professional. * Comfortable sharing your opinion (even if it is unpopular) * Ability to have open, honest conversations with patients and team members * Self-motivated * Believe in integrity in everything you do KEY RESPONSIBILITIES: (Job Description Upon Demand) * Handles confidential information requiring professional discretion and compliance with protected health information, data integrity and security policies. * Assists in maintaining patient records. * Confers with families, agencies, and pharmacies regarding patient care. * Provides oversight and assists in determining the needs of the patient. * Acts as a liaison between the medical provider and the patient for refill requests and other patient needs. * Manages emergency situations with patients in the office. * Supervises the process of prescription prior authorization. * Performs selected nursing and administrative duties and assists the physician as part of the clinical team. * Administers medication injections as needed and draws blood to check for certain levels * Other clinical or administrative duties as assigned to meet practice needs.
